Ebstein Anomaly || Ultrasound || Case 345

Clinical Features:
A 44-year-old primigravida came for a fetal echocardiography with the history of
Amenorrhea for 9 months
Normal 2nd trimester anomaly scan
Ultrasound Features:
Cardiomegaly (RA type) with a left sided axis deviation.
Apical displacement of the TV with a lowered coaptation point of TV into lower RV, not at the atrioventricular junction resulting in atrialization of the RV.
Tricuspid regurgitation.
Remember:
In absence of cardiomegaly & an upward directed apex, it may become challenging to diagnose Ebstein anomaly during the mid trimester scan.
Assess if there is flow across pulmonary valve.
