Using Gravity Forms (in my opinion it is the best WordPress forms plugin) - we will create and embed a form into our theme with PHP.

There are quite a few form plugins available in WordPress, though the best I've found is "Gravity Forms" - it allows for a lot of customisation and has a lot of extra features like integration with Mailchimp, Active Campaign and the ability to take payments.
