At the 1998 Goodwill Games, Sang Lan (gymnast in the first and last clip) made a mistake in a warmup Vault causing her to paralyze herself. Luckily, she is still alive to this day but is paralyzed. Unfortunately, many others have passed due to Vaulting accidents.

The Vault table changed shapes in 2001 due to safety concerns. The wider shape allows for a greater margin of error without compromising the block you get off the Vault.
